Some users have reported problems with survey input when using a smart phone. The issue has typically occurred with using the sliding scales on certain questions. There have been no reported problems when using a pad or laptop.
To facilitate reliable phone input, the survey has been modified to eliminate the sliders. They have been replaced with a series of number choices as is common in many surveys.
If you re-enter the site, you can finish any incomplete survey or start another.
We apologize for any confusion and inconvenience, but we hope anyone who gave up on the process will resume or complete another survey from scratch. Your opinion and input is needed.
